About your Role:

As a Family Group Decision Making Coordinator, you will work with children, youth, and families in conjunction with Children & Youth Services. You will support families in making a personalized family plan and provide written plan/notes to all participants and/or funding source within seven working days of family meeting. Organize and facilitate family meetings, attending any needed pre-conference meetings between agencies, and facilitate follow-up meetings as indicated in program guidelines. Schedule and complete pre-conference home visits with all families prior to first conference, and then as required by program policy. Provide updates to families and referral source as required by guidelines and effectively communicate with supervisors/managers, individuals, family members, referral source, and other interested parties.

Perks of this role:

  • Schedule is typically 8am to 4:30pm, M-F (is dependent on the families' needs)
  • $750 sign-on bonus- paid out: $250 after 90 days | $500 after 180 days
  • Competitive pay rate-$16.00-$19.00/hour

Does the following apply to you?

  • Bachelor's Degree in related field; OR
  • Bachelor's Degree in an unrelated field with a minimum of 12 credits in a Human Services related course; i.e. psychology, sociology, counseling etc. OR
  • Associate's Degree in a non-related field with minimum 2 years' experience in Human Service related field OR
  • High School Diploma with 5 years' experience in Human Services including experience facilitating meetings.
  • A valid Pennsylvania Driver's license with good driving record
