RN Plan of Care Coordinator Responsibilities:

• Demonstrate Appropriate Clinical Knowledge and Skills.
• Demonstrate knowledge of medical terms, medical abbreviations, medical charts, medications, and medical protocols.
• Demonstrate knowledge of ICD-10 CM Coding standards.
• Possess the ability to accurately review an OASIS assessment and identify inconsistencies in OASIS assessment items in accordance with OASIS guidelines.
• Comply with applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA and Documentation Standards.
• Demonstrate knowledge of the Medicare definition of Homebound status.

RN Plan of Care Coordinator Qualifications:

• Currently licensed as a registered nurse in the state(s) of agency operation
• Minimum of five years' experience as an RN; Minimum of two years' experience in home care
• Minimum of one year of coding experience (Certification in ICD 10 coding preferred at hire, required within 90 days); Minimum of one year of OASIS experience (Certification in OASIS preferred at hire, required within 90 days)
• Able to read, write and comprehend English
