We are currently seeking a talented, compassionate Nurses to support patients of the communities we serve. The Home Care Nurse role would primarily be located in the community and will be traveling to patient's homes and facilities. We have positions available at various locations for the following coverage areas: Baltimore City, Baltimore County, Howard County, Harford County and Carroll County. We currently have Full time, Part Time and As Needed positions available. .

Principal Duties and Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs-including physical, emotional and spiritual.
• Prepares and implements the care plan fo patient/family unit in collaboration with the Interdisciplinary group.
• Plans and supervises the delivery of care by the HHA.

Qualifications

• Current registration with the Maryland State Board of Examiners
• Associates or Bachelors Degree from an accredited School of Nursing required.
• Two years experience in an acute care, Hospice, home care, or long-term care setting
• Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
• Ability to meet the physical demands of the posltion with or without reasonable accomodation.
